Immunizations

  • Due to the increased incidence of vaccine preventable diseases, we have opted to only see patients who will adhere to the vaccine schedule for the safety of our patients.  

Well Child Visit Schedule

  • 1 week

  • 2 week

  • 1 month

  • 2 month

  • 4 month

  • 6 month

  • 9 month - hgb, hct, lead testing

  • 12 month 

  • 15 month

  • 18 month - hgb, hct, lead testing

  • 24 month - MCHAT

  • 30 month - MCHAT

  • 3 yr old and annually after 

Sick Visits and Referrals

  • All medical clearances and referrals require an office visit.  

  • The parent/guardian will be called 1-3 days  after the office visit with the contact info for the specialist.

  • All medications prescribed are sent electronically.  Please call the pharmacy 2 hours after your office visit to check if it is ready for pick-up.
